Registration Dates for QuadraMed 2008

Early bird registration for the conference runs until July 1, 2008. To qualify for early bird savings, your payment must be received by 12 a.m. EDT on that date along with your registration form. Pre-registration closes at 5 p.m. EDT on July 24, 2008. After that, all QuadraMed attendees may register on-site.

You can register in two ways:

• Electronically
  To register electronically, simply complete the on-line registration form. The on-line registration site works with Internet Explorer and Netscape, both V6 or above. If you encounter problems registering on-line, please contact Mandy Mann at Registration Systems Lab at +1 (407) 971-4451 or .

• Via Mail or Fax

  1.   Simply download or print the Adobe Acrobat .PDF file containing the registration form.
  
  2.   Submit the completed form being sure to fill the form out completely, either by entering your information electronically by clicking the "Highlight Fields" button in the upper right hand corner of the .PDF and then printing, or by printing the form first and completing it manually. Be sure to specify the payment method and any applicable credit card information.
  
  3.   Mail or fax the form and any payment papers (such as a check.)

Registration Terms and Conditions

Payment Methods Note: We must receive your full payment prior to the conference, or you will be expected to pay on-site in order to receive your conference materials. Acceptable methods of payment include the following:

• Credit cards - We accept MasterCard, Visa, and American Express.
• Company or personal checks - Your check must be mailed with your registration form. Keep in mind that to qualify for early-bird savings, we must receive your check by July 1, 2008. Please make your check payable to QuadraMed 2008.

Cancellation Requests - Refund requests received in writing and postmarked by July 23, 2008 will be processed minus a $50 administrative fee. NO REFUNDS WILL BE GIVEN AFTER July 23, 2008. If you do not cancel and do not attend, you will be charged the full registration fee. Substitute attendees are welcome at no extra charge; however, we request written notification prior to the conference for preparation of registration materials. For refunds, please allow two to four weeks for processing after the conference.
